could be a number of things, hard to guess without knowing your system specs.

It may be the drivers aren't installed correctly, old drivers, refresh rate too high for monitor, video options in game not correct or could be a hardware problem.

What you could try first is go to www.ati.com and download the latest drivers. Then uninstall your current drivers, run a program called Driver Cleaner http://www.majorgeeks.com/download.php?det=3214, and install the new ones and see if it works.

If not, please post some more details about your system.

just a thought, you are using the default windows drivers for your monitor, do you have any specific drivers for your monitor or are you able to download them? This could be the cause of the problem.

Also, are you using any refresh rate programs on your computer like reforce? If so, they could be forcing the game to use a refresh not supported by your monitor.

Did you try uninstalling your old drivers with driver cleaner and installing the latest ones?

Another thing to try is to use a refresh rate program like reforce http://www.majorgeeks.com/download.php?det=1847 to force windows to use a specific refresh rate supported by your monitor.
