Looking for a little insight/help

Is there a way for someone to fill out a form, receive a unique number (Req # for example) and have a folder made on a server with that unique number? I don't know how it would be done together. Separately, I think I can do it pretty easily, but automation would prevent typos/mistakes by the end users.

 

Depends what hosting client your using on your server. You must specify.
Are you talking about making an area of your site for support tickets or alike?
Also it would be allot easier if input was going into a specific file like a TXT document but then again I guess it would be just the same level of input required.
