only showing desktop wallpaper

I have been searching this forum and others and can't seem to find the fix.
My problem is that I only have my wallpaper on my desktop, no icons etc. I have read about this problem in other threads and on other forums but my problem is I can't pull up the task manager, use ctrl alt del etc. I can only use safe mode. I have done various scans and removed as much spyware as has been detected but I can't run my computer in normal mode. I am currently using win xp sp2.

Any help would be greatly appreciated, thanks in advance
