vidoe card issue

well recently ive had some serious computer problems dont know how they started and dont know how i fixed em but i did. but now i have some video card issues.

Cant Play Games
Cant watch video (when i can watch it, i cannot increase resolution)

these cause it to freeze
these are my dxdiag test results:
