HP P1006 laser printer question

How can I eliminate the page no., date and print source in the header and footer of prints?

 

Page setup
This is from within a program on my computer

This is from the page here at MajorGeeks, so anything off the internet
after page setup, select the Margins, Header & Footer tab

Under Title, use the down arrow and select blank
Under url, select blank
Under left, center and right change all to blank
(no title, no page number and no date)
