Nvidia Geforce 8400GS Blackout

I got a new Nvidia GeForce 8400 GS g-card and the screen blacks out i am running windows vista (pacard bell) the intergrated graphics work ok on highest settings so does the g-card but the second time i booted up my pc it showed all the stuff system components pc manufacture and the welcome loader but after that black screen nothing but a file browser it has done this for the third time i tried almost all i could think of.
I lowerd the grahpics to lowest settings, i adjusted screen resolution but it still boots up with a black screen and i am very confuse please help

 

Could be a component failure on the card or the fan is not spinning up.
if it is not the fan contact the card source as they have full warranty.
