    What could cause a hard drive head crash?

    Shock to the drive, wear on the mechinism causing the head to hit the platter during seek.

    also tiny specs of dust, which can in very rare occasions enter via the breather hole. As the heads just float above the platters any disturbance will send them crashing into the platters, makes a nice sound.
