Suddenly this error began with my GTX280 w/195 drivers, so I reinstalled 191.07 and it worked for a short period. Now with 191 the error returned.

I did a clean install of 191, using revo uninstall to make sure to get everything left of 195 and in safe mode I cleaned up with driver cleaner pro and driver sweeper. Any suggestions?

Check your system temperatures (esp the GPU) using HWmonitor. You can also use GPU-Z to get very accurate voltage and temp readings from your video card. If you're not sure what's considered too high, attach a screenshot of the HWmonitor window, and attach a log file from GPU-Z. Run GPU-Z in the background while gaming and it can give us a good snapshot of what's happening with your video card. The stock fan speeds on the GTX video cards are pretty low. I locked my GTX260 fan at 70% using RivaTuner due to high temps. Before doing this, it would idle at 55-60, and would hit 90c while gaming. Now it idles in the low 40s, and maxes at 65-70 after 2 hours of Call Of Duty4-MW. You might want to run some general system diagnostics too just to be thorough. You can download a free HD diagnostic from your HD manufacturer's web site, and a good free bootable RAM diagnostic can be found here.

Hey dlb, both gpu-z and riva tuner have me idling at 35-37c and in Crysis Paradise Lost both have me peaking at 64-65c. I'll get the other programs, but I didn't think heat was an issue but I checked to be sure.

Those temps are excellent. In addition to running the HD and RAM diagnostics, you should also run the System File Checker. First, you'll want to uninstall your video drivers and do not re-install them until all this is done. Put your Windows install disc in the drive and exit the window when it opens. Get to a command prompt:
WinXP: click Start, click Run, type cmd and press Enter
Vista/Win7: click the Start Orb; in the Search box, type cmd and at the top left of the Start menu you'll see cmd.exe and you'll right-click it, select "Run as Administrator".
Once you're in the black command prompt window, type sfc /purgecache and press Enter. This command cleans the System File Checker cache so if files need to be replaced, they will be pulled from the install disc, and not from the cache where they may be corrupt or otherwise faulty. The "purgecache" shouldn't take more than a minute or two (usually closer to 15-30 seconds). When it's done, type sfc /scannow and press Enter. This can take some time so be patient. If you get a message about needing to insert your Windows disc (it's already in the drive), just click "Try again". If you get this message repeatedly, and you have a 2nd CD/DVD drive, move the install disc to the 2nd drive and click "Try again". Once the SFC has finished, remove your Windows disc and install your video drivers. I hope this helps! Good luck!
    :-D

NOTE: in the commands above, be sure to put a space between the sfc and the slash / and NO other spaces; if you do not use a space before the slash, or you use a space anywhere else, the commands will not work and will simply error out with a "Command not recognized" type error. . . .

(if you're running WinXP, you can also perform a repair install as outlined in this article)

Okay, since non of my efforts proved successful with the newer drivers 191-195, I rolled back to my old 182.46s. So far all seems well. Just prior to the rollback I could not get STALKER:CS to stop freezing when loading a saved game, but now it's working.

Makes me wonder if "my rig" was completely screwed with the hype about "newest and better drivers" (notice I said my rig).

Before all the driver drama, Fallout 3 ran great, but with the "latest/greatest drivers" I just about abandoned that wonderful [when modded] game. Me thinks I shall give it another go!

    UPDATE:
I reinstalled FO3 with FOOK2 + MMMF3 + All DLC (except Mothership) and it appears that the old drivers are the answer. Nothing funky so far. With the newer drivers I was constantly troubleshooting and reinstalling.
