I thought once you visited a web site it updates or adds the icon. This use to happen when I had IE5 or maybe 6...have XP w/SP2 and IE7. Anyway, I go to a Yahoo site and figure the icon will appear when I close explorer and go back in, no luck. This normal?

Those little icons are called favicons. Some sites have them and some don't.
If you dump cookies or have a program that cleans up cookies, you lose the favicons.
