Forms Development and input storage etc.

Hi all,

I'm looking for a solution that I can run on an internal web base intranet that will allow certain users (admins) to design input forms and publish them to the internal servers. These links will then be added to our internal Wiki.

Other users would then be able to access this form, enter details, submit and have their input stored in a database or emailed to a central address.

Ideally free would be great and I'm keen for it to be MySQL and PHP based if at all possible.

Can anyone recomend something that fits the bil?
