EHR is Electronic Health Record, it's basically both are similar with EHR being the current favourite term but both EHR and EMR ( Electronic Medical Record), it's a form of digital medical record just like the paper based ones, can include images of scans and x-rays that the clinicians can review quickly, also some packages come with all you need to run a practice or clinic from patient booking, billing etc

In part the change from EMR and the reference to Medical was seen as a doctor thing whereas it's a full package in many solutions to cover aspects I mention above not just the clinical side of medicine but the administration side too
