how do i make this work

    i have done little work with html. but i am starting. i am making a site for pc repair and i need alittle help with a forum thingy. www.jritze.com/support to see what i have started. i want people to be able to fill out that forum according to what they have and i want them to be able to submit it and it sends it to me. I also want it to take them to a thank you blah blah blah page after they finish. who can help me?
