How would a person know if their computer has been taken over by someone else?

I've read before that sometimes hackers take over other people's computers and use them as a launching base to conduct their nefarious deeds. Now, this might be a dumb question, but how would a person know if their computer has been taken over and is being used by a hacker? And perhaps more important, how does a person free their computer from the hacker?
