Start bar tabs lack icons sometimes, as in if have firefox open, and its on my start bar, the little fox and planet symbol does not show. (happens with msword too)

Issue with surfing on a website, and suddenly another window i have open appears on top (windows 10 issue?) (requires video to really show)

Issue with typing on a website, and suddenly another window i have open appears on top and i am typing there. (requires video to really show)

Issue with playing a song in background, while typing an email, and suddenly my typing effects the window in the background and stops the youtube video from playing the song, despite the email screen still being on top. somehow the youtube screen underneath gets selected and stays underneath. I notice it gets selected because the tabs on the start bar flash and its selected. It gets selected on its own. I might as well keep my hands off the computer and watch as a tab flashes and gets selected even though its under another window.

Issue with windows flickering amazingly fast, i think between each other? impossible to show without making a video but i basically lose control of being able to do anything until it stops. Mouse still works of course. (requires video to really show)

Issue with having firefox windows open and when i go back to a window it's gone. It wont open when i click it on start bar even though it still shows. Further inspection reveals that the window was scrunched somehow? like minimize was overly applied to it? so all that appears is file, edit, view, history but no possible way to maximize it again.

^ALMOST THE WORST PROBLEM. But it happens often, if i have 5 windows open, 2 end up scrunched and i have to completely close them.

Graphics card and drivers seem fine.
Rolled back to previous driver, still have the problem. FYI I never had this problem back in december which is what the previous driver is dated.
    Disabled some settings,
Control panel, ease of access, ease of access center, make mouse easier to use, Enabled prevent windows from being automatically arranged when moved to the edge of the screen. (Changed nothing still have the problem)
Disabled windows 10 cloud, i don't need that crap (unrelated but just letting you know)
Disabled cortana (I dont need that)
Disabled most privacy issue settings (I don't need to send data to anyone, except you guys =p)
Disabled email login, I login with a password only.
Uninstalled every app that was uninstallable (I don't need windows store)
My graphic card is a GTX 760 nvidia geforce (terrible company the past 5+ years and especially when used with windows considering buying ATI if it solves my problems)
Went to these two sites disabled all the stuff there incase it was causing my problem (still have the problem)

Firefox was reset, this doesn't happen to firefox alone, happens with any windows open.
Firefox was also clean installed when I reset windows 10 installation.

By the way i still have the problem after already RESETTING my windows 10 installation, that means clean install (kept my files, like documents/pictures). Wondering if its still driver related? despite rolling back driver?
