Need a Program to Send Class Evaluations

Sorry if the title is not accurate. Here is what I need. I need a program that will allow the instructor to email a class evaluation to students so they can answer a few stock questions, offer feedback, and email it back to the instructor. Seems like it would be easy to find, but Google let me down.

The way they are trying to do it now is include the survey in the student's packet, and have them scan it, fill it out, scan it and email it back to the instructor. Yeah, everyone is going to do that.

Any ideas for one that is PC and Mac friendly? (for the student, the computer that will be generating the forms will be PC.)

Let me know if you don't find anything. A custom app can probably be built in java (which will cover both platforms).

I think I found something. What they need to do is email a survey link to students. Was going the long way around the problem.

Seems their approach made me take the long way as well. Instead of giving students a form to fill out, scan and email, why not just email the questions to them... Dooh!

 

Adobe Forms Central (part of Acrobat) does quite a nice job generating fillable forms. Not quite cheap though.
