I hope someone can help me find out if I have a true problem or not.

2 nights ago I downloaded a new scheduling app called Planning Center Service that an organization I'm in uses. I was added to the group by the organization leader and so I opened a link from my email from the app to go to their website, and from there a link to their Google Play store app page and proceeded to download the app. I entered my email and they emailed me a confirmation with a verification number to enter in the app before creating a password in the app.

8 minutes after receiving the confirmation email with the verification number, I received an email in my inbox with the password I had just created in the app saying they were a hacker and had hacked my email, my android phone, and knew 'everything' about me pretty much. They had all my contacts stored and were threatening to blast all of my contacts with reputation damaging things (I'm not as worried as you might think and I'm assuming this is not information they have actually gained or are able to do). Using all the scare tactics of needing to send them bitcoins within 48 hours, don't call the police, there is nothing I can do (even changing passwords won't help, etc.

Since then I have been updating many of my passwords across the accounts I can think of and I went through the READ ME virus removal on this forum on my desktop just in case I have something on my computer too. I have attached my log files. I noticed Adw Cleaner found 4 PUP files and only deleted 3. Can anyone verify that my PC is fine from my logs below?

I would also appreciate any advice on how to check/clear anything from my phone.

Finally, what are suggestions for preventing hacks and viruses on both my PC and Android phone for the future?

    Thanks in advance!

Please go to add/remove programs and uninstall Covenant Eyes.

Reboot and run the C:\MGtools\GetLogs.bat file by double clicking on it (Note: if using Vista or Win7,or Win8 or Win10 don't double click, use right click and select Run As Administrator).

Is that what the last PUP file is that didn't get deleted? I have Covenant Eyes software installed intentionally so if that is the only thing showing up that is good.
Any advice on how to check whether my android phone has any malware?

No, I believe that is the cause of your issues. Please uninstall it and then get me a new MGLogs.zip.
