PC keeps crashing every once in a great while.

I keep getting this odd issue where my pc will lose picture and my audio will loop, i dont get a bluescreen but i do see errors when i look at eventviewer. I personally have been dealing with this on and off for several months, and i thought i resolved it.... but within a week i got another string of black screen's, and its odd i usually get them maybe once a week but when they occur they happen 2 or 3 times in one day before they end up going away for another week. The bugcheck was: 0x00000116 (0xfffffa8020c594e0, 0xfffff8800fe5a7a0, 0xffffffffc000009a, 0x0000000000000004). I have 5 other reports, the only difference is the first parameter, everything else matches up.

My PC - Intel Core i7 2600k, Gigabyte Motherboard Z77X-D3H, Geforce GTX 480, 32 gb Corsair Vengeance Ram.
