Let's start with basics here... have you uninstalled and reinstalled the driver? How about game? Which operating system are you in?

What else do you have running?

Also, have you tried another driver? Update to the latest driver if you haven't already. If you have the latest one, roll back to a previous one. The latest and greatest driver is not always the best supported one in a game.
