Im currently doing a case study whereby I have to rollout a new email system in a hospital and I would like some advice/feedback on the subject.

At present the hospital is using a paper based memo system and want to upgrade to an email systems.

The backbone of the network comprises of Novell, Linux and IBM boxes.

I will have to purchase Mail server and 350 new machines, mail software etc.

What would be my logical next step in looking at the mail server software and machines?

Talk to Novell. You'll get support and service from one of their reps. It's nowhere near professional enough to be giving advice for this kind of project on a random Internet forum.

    Welcome to the 1990's.

da chicken is right, you need to seek help to spec out a job of this calibre, but in the interest of letting you know all the options available to you. As well as Novell, you could look for someone offering Exchange Server installation (based on Windows Servers with Active Directory) or there are Linux based solutions as well, such as Scalix and Zimbra among others and Apple Mac also have a Mail solution built into their OSX Server software.

All of these solutions will need a fair amount of knowledge to get set up and running properly so unless you have a spare server and a lot of time on your hands to play with them to get up to speed, I would definitely get someone to do the work for you.
