Strange Texture Problem

Hi,
I won't make any miles long intros, does anyone know what causes a problem seen on the screenshots attached? (GTA IV - obvious, all over the place, Skyrim - grass in front of the character). I have already disconnected my GPU and tried intel HD 4000, issue is still there, could it be some software missing?

 

Soz for low detail, I hoped it's known problem 'X' thats solved by doing 'abc'
Well the gpu is MSI's gtx 570 power/OC.
I tried it at my OC (900/2100), their oc (770/2000) and reference (732/1something), problem is still there.
The error is there regardless of playtime, the temps are not exceeding 70 on both gpu and CPU (i7 3770k, TJ max 105, OC'd to 4.4)
The issue is there regardless of driver version (tried 301.42 and 285.xx, recommended by MSI support)
DX 11 ofc
and so far I have only spotted this error in GTA IV and Skyrim.
Also, as I said, I have already taken that gtx 570 out, and tested the game (skyrim) with Intel HD 4000, with low settings, the problem is still there, so I don't think its strictly the gpu ;/ (I can try modding with some extreme textures to see if it changes)
Other than that I have 8 gigs of Corsair Vengeance at stock clocks (9s and 24), Asus P8Z77-V mobo, XFX 850W power supply.
Imma try lowering CPU OC and check whether it helps, but im in school for the next hour (though, the temps during prime95 dont go over 85).
Lastly, I have/had them installed on Vertex 4 128 SSD, and there's a firmware update available, so could that be a problem?

 

Could have been overheating and/or throttling
As for the updates, I'll check directx, and already looked up any significant updates in windows. I'll try nvidia 296 drivers as well.
As I said, I figured out it's not the gpu itself, since bug is on both GTX 570 and Intel HD 4000. And downclocking the cpu to 4ghz didn't change anything. I'll try installing GTA on normal drive and see if its vertex's fault.

 

Well what version of GTA do you have do you know there is a 1.4 version update i can link.

Do you have a store bought CD or is this offline download of GTA 4

 

GTA IV installed on HDD (caviar blue). patch 1.0.7.0
Max settings, using less than a gig of VRAM on max settings (night scene though)
working correctly.
Might re-install skyrim and check thursday after maths exam.
Conclusion so far:
If you have rainbow-like squarey texture artifacts in game, re-install on a different drive.

Will attempt updating firmware on that SSD then - also gonna prolly happen thursday.
