Survey keeps popping up in browser. How do I get rid of it?

    Recently, when on the internet a survey keeps popping up saying I won something and asking me to complete the form. I am smart enough not to fill it out and keep closing it down, but I am getting annoyed with it. How do I get rid of it for good?!

    I have windows 7 and a dell vostro 3700 laptop. For a browser I usually use Goggle Chrome.

    Oh, and it shows up when I go on several different websites. It is not just limited to one specific one. Can any one help me with this? Thanks.
