Template Creator Windows: Added File Type Doesn't Appear

Hi! I have a program called Template Creator. It is my own creation. It is a program to create, handle, organize and use template files. I have versions for DOS, Windows and the C64 at tmpcreat - Manage Files at SourceForge.net. The C128 version currently doesn't work. Earlier today, I tried to associate .dsk files (Apple2 disk images) to it, and the entry doesn't appear in the ShellNew menu. I noticed that, if a file type doesn't have a name, it won't appear, but it has a name. I tried logging off then back on. It still didn't appear. I can attach a Registry dump here if you want.
