    Hi

Is that a cable end or is this a graphic card endplate if the latter what Video card make and model is it off as that will make an ID easier?

It doesn't to me look like a standard DVI-I, DVI-D or DVI-A cable, holes are different layout, although in some cases that means nothing as some holes have no wire attached, the one thing we do know is that it has no slot, so rules out DVI-D

Bingo - a little image search gives up this HERE a DMS-59

It requires a special Y-cable to split between 2 to 4 connectors. We have these for our dispatch (911) computers at work.
