You don't have the welcome screen turned on -

Start > Control Panel > User Accounts > Click 'Change the way users log on or off' > Make sure the Welcome screen box is checked.

First problem can be solved by following these steps:

    1. Open Control Panel
    2. Click the 'User Accounts' link (or double click the icon)
    3. Uncheck the box next to 'Users must enter a user name and password to use this computer'
    4. Apply your settings.

Second can be solved by following these steps:

    1. Right-click the start menu
    2. Select 'Properties'
    3. Select the 'Start Menu' tab
    4. Select the 'Start Menu' radio button
    5. Apply your settings.
