DSL Modem Possibly Failing

Hi MajorGeeks,
I have been using a Westell 327w DSL Modem for nearly five years. It is connected to only one desktop PC using the Ethernet connection. Wireless service is disabled. After nearly three weeks of contacting my ISP, Verizon Online, with complaints of constantly intermittant DSL service and very slow (about 350 kbps down to 25 kbps) internet speed, I was told by a "technician" earlier this week that his "best guess" was that the modem is failing. However, no tests performed have proved this. I am attaching a diagnostic log from late last night during a time this activity was occurring. If someone could please review the log and help me to understand what is taking place, I would appreciate it.
