Ive Been Infected, Need Help

I am new to this board, & have followed all the directions to my best ability.
The free dl's & links are great, thank you.
Attached is my HJT file.

I was having major issues with:
NEWDOT~2
also: limewire & other apps kept trying to boot up (phantom boot ups)

Yes, I probably got infected using Limewire, undoubtably while looking for that 1 legal backup copy to my CD collection

Ive got it working OK in safe mode, after much msconfig manuvering & manual deleting of .exe files in windows directory. Id really like to know

1. How do I get clean?
2. How can I stay clean & not spend 8 hours on this again?

Any/All help is greatly appreciated.
