Well , The Symptomps you have told indicate you have a Optical Mouse, If Yes, then change the Mouse Pad, There is a Issue with Optical Mouse because of the basic Technology of how it works, when you change the mouse Pad, please ensure the Mouse Pad is not shiny and reflective, better go for a Dull, Rough, Opaque Surfaced Mouse Pad.

Is it a Ball PS/2 Mouse or a Optical USB Mouse ? If that is an old Mouse then its unlikely to be a Optical Mouse, May be its days are over !!!! Try Removing the Ball, Clean the Ball, Clean the Rolers within and try
