Need a programmer for a small task

Hi,
this is Bill Kelso Mfg maker of leather jackets
we are looking for a programmer to create an order form for our website,
The current one can be seen at our website www.billkelsomfg.com
if you click "Order Here". This one is very simple and faulty.

The one we need is a little more sophisticated than the existing one.
We need the product to be chosen and then the various options and then the form should calculate the total etc.
