Outlook 2019 Keeps Prompting For Password

My Outlook 2019 keeps randomly popping up a prompt every so often for the e-mail password, even though I already know I have all the right settings and password because all my e-mails come through just fine. Here’s a screenshot. So how do I trouble shoot this and make it stop doing this?

One weird thing is that Outlook somehow appears to have automatically changed my e-mail server names because those aren’t the server names I put in when I first set up the accounts. It was actually confusing at first when I was setting up the e-mail accounts, because Outlook wasn’t exactly clear on how / when / where to enter the incoming and outgoing server names. Does Outlook somehow know how to automatically figure what the server names and port numbers are supposed to be? And how come it keeps asking for the password again and again?
