Please help!!? Malware Already Found And Still Problems!

Hi,
I am trying to fix my parents home computer while I am here. I really don't know what to do - there was a virus that I think i've managed to get rid of but there are so many things I think that shouldn't be loading on start up but I have no idea where to begin.
I am only going to be here for for another 18 hours. Can someone please help me in that time? It is running very slowly and apparently has been like this for some months now but I don't live even in the same city or state and need to fix it as best I can.

I have run full system scan with Ad-Aware.
