Should I be concerned

Instinct says "Yes" I should be concerned, my ad-aware found this:
win32.trojandownloader.agent.de. Ad-aware considered it "low risk". I quarantined it. Hmm, what should I do? My AVG antivirus updates frequently..shouldn't it have caught any trojan?

After doing ALL of the above you still have a problem make sure you have booted to normal mode and run the steps in the below link to properly use HijackThis:
