win472.tmp, win32.silentcaller.z and so much more

Hi, I am having a major problem with trojan horses, viruses and malware. I've followed all the instructions on how to get rid of them from various sorces including some postings from this forum and just when I think they might be gone, they come back. The current favourites today that keep popping up are win32.SilentCaller.z, win472.tmp, and something with silly in the name. Please someone help me kick this thing for good. Here is my current HJT log.
